Q: I am seeking an example of someone’s release of liability for College for Kids or similar summer camp adventures.

A: Yes, we have some examples from different types of organizations and I have attached two for your reference.  One is a PDF that includes Liability as well as Medical, Emergency Contact and Registration forms (Reston Community Center) and the other is a Liability form from SUNY (State University of New York).  Please let us know if any other information will help.
