Q. We would like to send out a survey to our Informal Classes customers that have only taken one class with us to ask, in the best way possible, why they have not come back to take a class with us again. We would like to find out why they are not repeat customers.
A: I have attached a sample of a possible survey you might use. Of course, changing the questions might suit your needs better, but this will give you an idea.
