Q: We are hoping to follow the evaluation format that you would recommend I believe you use a 5 point rating scale can you send me the specific language and format that you would recommend, as we move forward with it implementation I want to make sure we are using your best practices.

They are formatted differently, but pleasenNotice that each isreader-friendly with a clean, professional look and a limited number of questions, and both use a 5-point scale. It is immediately clear that the evaluation form is short and easy to complete, which is important.
