**Q: **We would like to incorporate a class survey form (like survey monkey) in our website, and as a link we can include on receipt (and perhaps we can send an automatic response after the class is over.)

Do you have recommendations on a survey form or do you recommend we use survey monkey link?

**A: **We use survey monkey for this. We initiated a discussion on LinkedIn for this question. I will invite you to join the LERN group if you are not already a member. Once you are a member of the group, search for evaluations. The programs members indicated using were:

Survey Monkey
Qualtrics
Lime Surveys
Class Climate

The most commonly used resources was Survey Monkey.
